You must set the ad_network_ads.txt file to be writable (check file name as well).

What kind of pain is it? I always recommend going to an ENT if it's something that lasts more than a day or so. Go read http://ronfez.net/forums/showthread.php?t=73042 to see why, long story short I had a minor issue turn into severe tinnitus and degenerative hearing loss.
