Oldboy.
Totally agree. It's the most fucked up, personal revenge plot ever thought out.
And it totally caught me off guard.

Primal Fear

Good call! I would say I didn't see the twist coming, but when I was dating Mrs. ANC, she asked me if I ever saw Primal Fear. I said no. She said oh you have to, she gives me the premise then says "And at the end of the movie....(spoiler)"

I said "Thanks now I don't have to see it."

Shawshank Redemption also had a great twist.

How would that be a joke? That movie has one of the most ambiguous mind-fuck endings of all time.

At the end of Night Nurses 2, it turns out all the sex he had reversed his once thought to be deadly illness. He is cured and ends up checking out of the hospital. Couldn't believe it.

I don't know if it's the best twist ending, let alone a twist, but I thought Vertigo had a great ending.

If we are going to go Dexter I think the end of this Season buries any of them.

Its funny, I caught this movie the same way. And I LOVED it. One of my favorites.

If I had to pick one, the one that blew my mind the most was the end of Sleepaway Camp. Maybe because I was a kid but it just blew my mind.
