You must set the ad_network_ads.txt file to be writable (check file name as well).

You can trial a bunch of them. Try DVD Audio Extractor.

FYI - Some DVDs are really tough to work with - I had a hell of a time finding an app that worked with a Live David Gilmour DVD. If all else fails you can play the DVD and "record what you hear" with Audacity (free.)

Yep - it's (Audacity) a good last resort - Xilisoft makes good ripping/converting stuff.
