The secret is basically an allegory for Fez's happiness. Fez's happiness is predicated on his being the pathetic center of peoples' attention. Fez loves being unhealthy and being depressed, because it gives him something to talk about.

Because Fez loves his misery he can't and won't tell his family his secret. If Fez wanted to get better he'd stop trying to diagnosis his problem or find medication that would cure his "problems" because his problems are within his psyche.
