Hi Dr.Steve,
I occasionally get a strange pain in my back. It always occurs around the same place in my lower-mid back on the left hand side, in a golf ball sized circle, and feels like someone is burning me with a hot poker. It pops up more when I'm sitting than anything, and I feel like it is more frequent when I'm not getting a lot of exercise (although I've got it now, and I've been good lately). I always assumed it was just regular muscle pain, but the burning/localization makes me wonder if it could be nerve related. Should I be concerned, or just get some bengay.

Anytime you get a "burning" pain, you wonder if it's nerve related. "Regular" pain from trauma or tissue damage generally is described as either sharp or dull, stabbing or throbbing. "Pins and needles" or "numbness" or "burning" or "electricity" are all signs of pain that's generated by nerves being compromised.

You may simply have a pinched nerve as it exits the spinal column...

You may simply have a "trigger point", which is thought to be a little localized inflammation that can be treated with a shot of cortisone...

You may simply have a localized muscle spasm that's compromising a superficial nerve...

you may simply...well, you get the picture. It's hard to say much more without poking on it and giving it a look. If it bugs you, get it checked out. It's amazing what a shot of cortisone can do for stuff like this sometimes. :-)

your pal,
