dr steve,

i am a 29 yr old man and a couple years ago, maybe 3 yrs or so, i started feeling a dull aching pain in my left testicle. then shortly after that i found a lump on the side (if they have sides) and had it checked out. the first doctor said it was normal, something to do with the tube that carries semen blah blah blah. i didn't like that response so i went to see another doctor. he actually did some tests and told me it was a vericocele. i had the corrective surgery almost exactly 2 yrs ago. for a long time after that the testicle was extremely sensitive, a hard step or even running would cause a lot of pain. now it is still very tender when anything comes in contact with it, the lump is still there and i believe its gotten bigger and from what i can see the left testicle is a little bigger than the right. it still functions normally, as far as ejaculation goes (i noticed a decrease in volume, may or may not be related) and 9 times out of ten i feel the need to urinate shortly there after. i know they siad a varicocele can come back but i'm starting to doubt that was the problem in the first place seeing as the lump never went away. however, the dull ache i felt before hasn't returned since the surgery. is there anything else this could be?

It's strange, a varicocele is usually described as feeling like "a bag of worms" in the scrotum...let me see if I can find a picture of one:

Basically, there are these veins in the scrotum that carry heat away from the testicles. The testes work better if they're a few degrees below body temperature, and the vein complex acts like a radiator or air conditioner. If the veins lose their integrity (by taking a bribe, perhaps? yeccch.) they'll become "varicose veins", big, loopy, bloated veins that no longer work the way they were supposed to.

So I am also a little concerned about the original diagnosis, though once the surgeon got in there he/she would know...since they didn't say "hey, we were wrong about that" I'm going to assume that there WAS a varicocele, but that there's something else going on too.

After surgery, you can indeed develop more varicose veins, but also you can get a few other post-surgery things including a "hydrocele" (a clear fluid collection), a "spermatocele" (a not-so-clear fluid collection), and other things. Also, check out the thread on epididymitis (http://www.ronfez.net/forums/showthread.php?t=71257&highlight=epididymitis), which is what your original doctor thought you had.

What I'd do is go see a urologist; he or she will get an ultrasound and see what's going on. In the meantime, scrotal support is the key...if you wear boxers, move to briefs...if you wear briefs, try an athletic supporter.

Damn, dude..I don't know how you can put up with it. I hate when my "prostrate" hurts.
